Ask About Your Records
Contact the medical records team to ask what is held about you, request a copy, ask who has accessed your file, or raise a concern about how your information has been handled. We will explain the process, what you will receive, and what the applicable local regulation requires of us.

Visit
301 Huafu Avenue North Section, Jiulongpo District, Chongqing
Near Huayan Station, Metro Line 5 (about 500 m south)
Records requests are handled at the hospital. Bring identification, and expect to sign a request form that records what you asked for and when.
Written Requests
Put a request for records, a correction, or a concern in writing, and keep a copy of it.
A written request is dated, acknowledged and answerable. An oral answer cannot be checked later, which is why we encourage patients to follow up in writing rather than by telephone.
